Azure CVO node offline with "Entering FM state:5" error on serial console on NIC detach

Issue

After a reboot, a CVO node becomes unavailable. If you log into the serial console or view the serial logs, the following message is found multiple times:

Entering FM state:5 because mbFound:0 local in headswap:0
